Combine in 3-quart slow cooker: whole milk, half-and-half, chopped chocolate, sugar, cocoa powder, vanilla, espresso powder (if using), and salt.

Whisk mixture thoroughly for 1-2 minutes until cocoa powder dissolves completely.

Cook based on setting: Low for 2-3 hours, or High for 45-60 minutes. Whisk every 45 minutes.

Once hot and chocolate is melted, keep on low or warm setting, ladle into cups, and add desired toppings.
